Not all HRT is the same, and the route it enters the body turns out to matter enormously for certain women's safety profiles. The conversation has shifted significantly in recent years as researchers have unpacked exactly how oral and transdermal estrogen behave differently once inside the body. Understanding these 11 differences can help women and their clinicians make a genuinely informed choice rather than a default one.

Venous Thromboembolism (VTE) Risk Is Substantially Lower With Transdermal Estrogen

Oral estrogen passes through the liver during first-pass metabolism, triggering increased production of clotting factors such as Factor VII, Factor X, and fibrinogen — a process that raises VTE risk by approximately two- to threefold. Transdermal estrogen bypasses the liver entirely and does not meaningfully alter these clotting proteins, leaving thrombotic risk largely unchanged from baseline. Multiple large observational studies and meta-analyses consistently support this distinction, making transdermal the preferred route for women with any personal or family history of blood clots.

First-Pass Liver Metabolism Affects Oral Estrogen But Not Transdermal

When estrogen is swallowed, it is absorbed through the gut wall and travels directly to the liver via the portal vein before entering the systemic circulation — this is called first-pass metabolism. The liver then processes and partially inactivates the estrogen, meaning a higher dose must be taken orally to achieve a therapeutic blood level compared to the same hormone delivered through the skin. Transdermal estrogen absorbs directly into systemic circulation, reaching target tissues without this hepatic detour and producing a more stable, lower-fluctuation hormone profile.

Oral Estrogen Raises Triglycerides; Transdermal Does Not

The liver's exposure to supraphysiological concentrations of estrogen during oral administration stimulates triglyceride synthesis, which can elevate fasting triglyceride levels — a concern for women who already have hypertriglyceridemia or are at cardiovascular risk. Transdermal estrogen bypasses this hepatic stimulation and in some studies has been shown to modestly lower triglycerides, or at minimum leave them unchanged. For women with elevated baseline triglycerides, this difference is clinically meaningful and worth discussing explicitly with a prescriber.

Blood Pressure Response Differs Between the Two Routes

Oral estrogen stimulates hepatic production of angiotensinogen, a precursor in the renin-angiotensin system that helps regulate blood pressure — in susceptible women, this can cause a measurable rise in systolic blood pressure. Transdermal estrogen does not significantly stimulate angiotensinogen production and has a neutral or mildly beneficial effect on blood pressure in most studies. Women with pre-existing hypertension or borderline high blood pressure are therefore generally better candidates for transdermal delivery.

Sex Hormone-Binding Globulin (SHBG) Is Raised More by Oral Estrogen

The liver responds to oral estrogen by producing more SHBG, the protein that binds sex hormones in the bloodstream and renders them biologically inactive. Higher SHBG levels can reduce free testosterone availability, which may worsen low libido, fatigue, and mood in women who are already testosterone-deficient in perimenopause and menopause. Transdermal estrogen causes only a modest rise in SHBG, meaning more free testosterone remains available — a practically important difference that is often overlooked in prescribing conversations.

Stroke Risk May Be Lower With Transdermal Estrogen

The same hepatic clotting-factor changes that drive VTE risk with oral estrogen are also implicated in ischemic stroke risk, and observational data suggest oral HRT users have a modestly elevated stroke risk compared to non-users. Studies examining transdermal estrogen have generally found no significant increase in stroke risk, even at higher doses, likely because the clotting cascade is not activated in the same way. This distinction is particularly relevant for women with additional stroke risk factors such as migraine with aura, hypertension, or obesity.

Bioavailability Is More Predictable With Oral Estrogen — But Not Always Higher

Oral estrogen tablets produce consistent pharmacokinetics that are well-characterised in clinical studies, making dosing straightforward to standardise across populations. Transdermal absorption, however, varies between individuals based on skin thickness, application site, hydration, body temperature, and whether the patch or gel is applied correctly — meaning serum estradiol levels can vary considerably between women on the same nominal dose. This is why symptom monitoring and occasional blood level checks can be more important with transdermal delivery to confirm adequate absorption.

Migraine With Aura Is a Specific Contraindication That Favours Transdermal

Migraine with aura is associated with an independently elevated ischemic stroke risk, and combined with oral estrogen's prothrombotic effects, the combination is considered sufficiently concerning that many clinical guidelines caution against oral estrogen in this group. Transdermal estrogen, by avoiding clotting factor changes, does not carry the same additive risk and is generally considered the safer option for women with aura migraines who need HRT. Some women also find that the more stable serum estradiol levels achieved with transdermal delivery reduces hormone-fluctuation-triggered migraine frequency.

C-Reactive Protein (CRP) and Inflammation Markers Respond Differently

Oral estrogen has been shown in multiple studies — including a substudy of the Women's Health Initiative — to increase high-sensitivity CRP, an inflammatory marker associated with cardiovascular risk. Transdermal estrogen does not appear to raise CRP and in some studies modestly reduces it, suggesting a more favourable inflammatory profile. This difference may partly explain why the cardiovascular outcomes seen in older oral HRT trials have not been replicated to the same degree in studies using transdermal delivery.

Gallbladder Disease Risk Is Higher With Oral Estrogen

Oral estrogen increases biliary cholesterol saturation through its effects on hepatic cholesterol metabolism, which raises the risk of gallstone formation and cholecystitis — the risk is estimated to be approximately two- to threefold higher in oral HRT users compared to non-users in large cohort studies. Transdermal estrogen has a significantly attenuated effect on bile composition because it avoids the high hepatic concentrations that drive this change. For women with a personal history of gallstones or known gallbladder disease, transdermal delivery is the more appropriate route.

The Progesterone Component Also Matters — Micronised Progesterone Has Its Own Route Advantage

While much of the transdermal versus oral debate focuses on estrogen, the progestogen component carries its own delivery nuances — oral micronised progesterone (body-identical) appears to have a more favourable cardiovascular and breast risk profile than synthetic progestins when used alongside either route of estrogen. Crucially, transdermal progesterone creams do not reliably achieve sufficient uterine protection and should not be used as a progestogen for endometrial protection in women with a uterus — this is a critical safety point that is sometimes misunderstood in wellness spaces. The safest combination for most women with a uterus is transdermal estrogen plus oral micronised progesterone, which represents the lowest-risk evidence-based regimen currently available.
